We are seeking an RN/LPN to join our team! You will be responsible for the assessment, diagnosis, and treatment as ordered of assigned patients.
Responsibilities:
To provide nursing care in accordance with the Client s plan of care to include comprehensive health and psychosocial evaluation, monitoring Client s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care
Coordinates total client care by conducting comprehensive health and psychosocial evaluation, monitoring client s condition, health promoting sound preventive practices, coordinating services and teaching and training activities.
Must be a graduate from an accredited school of nursing and licensed as a registered or practical nurse in North Carolina
Minimum of one year experience

- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients and family about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records, submit clinical and progress notes timely, and presents to clinical record review
- Provide advice and emotional support to patients and their family members
- Participates in Client s case conference and discharge planning process
Must be a graduate from an accredited school of nursing and licensed as a registered or practical nurse in North Carolina
Minimum of one year experience
- Previous experience in nursing or other medical fields
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
