  • August 1-5, 7am - 7-pm
  • August 15-19, 7am - 7pm
  • August 27 - September 2 7am - 7pm
  • September 10 - 16, 7am - 7pm
  • September 26 - 30, 7am - 7pm
  • Call is 7am - 7am

This could be split between multiple providers, please specify which dates you can cover Support Staff: Nursing End Date: Ongoing (could be considered for Perm Position) Coverage: Scheduled Clinic Hours + Call (Beeper); Rounding of Assigned and Unassigned Patients Requirements: Board Eligible or Board Certified; Must have active AZ license; Clean Malpractice, DEA, and NPDB;Fellowship preferred Certifications: ABLS, ACLS, ATLS, BLS, PALS Travel and Lodging provided Malpractice Insurance provided
