Provides personalized education, training, and support to patients with diabetes or prediabetes and other chronic diseases, including hypertension and COPD focusing on self-management behaviors. Develops and delivers diabetes and other chronic disease education plans, teaches lifestyle modifications, and instructs patients on glucose monitoring, medication management, proper nutrition, and exercise.

We were one of the five clinics which founded the Texas Association of Charitable Clinics. Generous support from grants and donations allows us to provide primary patient care to those who meet our eligibility requirements. Come join us and our Mission which to minister in a caring and Christian environment to the medically underserved of Kerr and surrounding counties. Everyone deserves quality care.
