Position Summary:

We are seeking a compassionate, organized, and dedicated Licensed Practical Nurse (LPN) to join our team in an Assisted Living (ALS) community. In this role, you will be instrumental in conducting resident clinical assessments, providing on-call clinical support during off-hours, and serving as a welcoming ambassador by giving community tours to prospective residents and their families.

Key Responsibilities:

• Clinical Assessments: Conduct initial, routine, and change-of-condition wellness assessments for current and prospective residents to determine appropriate levels of care and develop personalized service plans.

• Tours & Community Relations: Provide engaging, informative tours to prospective residents and family members; answer questions regarding care levels, services, and daily life in our assisted living community.

• Off-Hours Triage & On-Call Coverage: Respond to off-hours phone inquiries, provide telephone triage, and answer facility on-call pages to advise floor staff, coordinate physician orders, and handle urgent resident care situations.

• Care Coordination: Collaborate closely with resident care aides, physicians, pharmacies, and family members to ensure continuity and quality of care.

• Compliance & Documentation: Maintain thorough, accurate clinical documentation in compliance with state regulations and community policies.

Qualifications:

• Current, unencumbered Licensed Practical Nurse (LPN) license in the state of Georgia.

• Minimum of 1-2 years of nursing experience, preferably in Assisted Living, Memory Care, Long-Term Care, or Geriatrics.

• Strong assessment, critical thinking, and clinical triage skills.

• Warm, professional interpersonal and customer-service skills with confidence in giving tours and speaking with families.

• Reliable availability for scheduled off-hours on-call rotation and phone triage.

• Current CPR/BLS certification.
