Home Health LPN - 13-Week Travel Contract

KPG Healthcare is seeking an experienced Home Health LPN for a full-time 13-week travel assignment. The LPN will provide skilled nursing care to patients in their homes, completing scheduled visits and managing care based on each patient's individual needs.

Responsibilities:

• Provide skilled nursing care to patients in their homes
• Complete scheduled in-home patient visits and provide care based on the plan of care
• Monitor patient condition and report changes to the RN or supervising provider
• Administer medications and treatments as ordered
• Perform wound care and other skilled nursing procedures as needed
• Reinforce patient and family education and care instructions
• Communicate with the RN, physicians, and other members of the healthcare team
• Document visits using Homecare Homebase (HCHB)

Requirements:

• Active LPN license in the state of practice
• 1+ year of recent Home Health experience
• BLS certification (AHA)
• Homecare Homebase (HCHB) experience preferred
