Nurse Practitioner

The position features a flexible schedule with a mix of in-office and telehealth appointments, requiring strong communication skills and a patient-centered approach.
Responsibilities
- Conduct patient evaluations and assessments
- Diagnose and treat common adult health conditions
- Manage chronic diseases and ongoing care plans
- Provide timely follow-up care and patient monitoring
- Educate patients about health management and treatments
- Coordinate patient care with other healthcare providers
- Deliver telehealth services efficiently and effectively
- Document patient visits accurately in medical records
Preferred Qualifications
- Entry-level experience in nurse practitioner practice
- Proficiency in patient assessment and clinical diagnosis
- Experience managing chronic diseases
- Familiarity with electronic health records (EHR) systems
- Comfortable using telehealth technology
- Strong patient education and communication skills
