Geriatrician Opportunity - PACE Program Greater Denver Region


This is a rare opportunity to join a highly collaborative care model supporting medically complex, dual-eligible seniors to remain safely in their homes and communities.

You'll begin at the flagship site with full training and panel development before transitioning into a new Denver center with a clear pathway toward future leadership as Associate Medical Director.

Clinical Model & Daily Work
  • 1-5 daily face-to-face encounters with complex older adults (55+ dual-eligible)
  • Extended visits (45 minutes to 2+ hours) focused on comprehensive care coordination
  • Territory coverage across 7 skilled nursing facilities in Boulder and Weld Counties
  • Full participation in interdisciplinary team (IDT) huddles, care planning, and daily coordination workflows
Call & Support Structure
  • 100% telephone-based call from home (no inpatient rounding)
  • Strong nurse triage system filters non-urgent calls
  • Call is fully compensated and shared across providers
Candidate Profile

We are seeking a collaborative, mission-driven physician who thrives in relationship-based geriatric care.


Must have:

  • MD or DO with completion of Internal Medicine or Family Medicine residency
  • Fellowship training in Geriatrics or Hospice & Palliative Medicine strongly preferred
  • Comfort managing medically complex, frail, dual-eligible older adults
  • Interest in SNF-based care, PACE models, or safety-net populations

  • Guaranteed base compensation of $300,000 plus call pay
