- Manage outpatient patients with solid tumors and hematologic malignancies in a supportive environment
- Collaborate with hematologists/oncologists, infusion teams, nurse navigators, and support staff
- Support patients with chemotherapy, immunotherapy, and supportive care therapies
- Utilize Epic EMR to document and coordinate patient care
- Provide patient education, symptom management, and follow-up care
- Support goals of patient continuity and relationship-driven care
- Assist with care coordination, prepare for procedures, and participate in clinical trial processes as applicable
- Active Minnesota license, with current certification as PA or APP
- 2+ years of outpatient hematology/oncology experience preferred
- Ability to work independently and collaborate within a multidisciplinary team
- Strong communication skills and compassionate patient-centered approach
