Field Nurse Practitioner
Locations: Boise, ID; Palm Springs, Bakersfield, Fresno, CA; Middletown, New Haven, Litchfield, Willimantic, Rockville, New London, CT

Molina Healthcare is seeking experienced Nurse Practitioners to provide in-home and community-based care, focusing on preventive services and health screenings. Work environments include patient homes, nursing facilities, and pop-up clinics.

Key Responsibilities:

• Conduct comprehensive medical assessments

• Provide care coordination and treatment planning

• Perform face-to-face visits (home, SNFs, community sites)

• Use Epic EMR in the field

• Travel within a 60-mile radius; up to 120 miles occasionally

• Attend mandatory 5-week training

Requirements:

• Must be comfortable working autonomously in non-traditional settings

• Able to manage diverse patient populations

• Strong documentation and diagnostic skills

Schedule:

• 45 minutes per visit

• 6 9 visits/day depending on shift

• No follow-ups or ongoing care
