What You ll Do

• Independently provide comprehensive outpatient primary care to diverse patient populations
• Focus on health promotion, disease prevention, and wellness education
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
• Conduct assessments, diagnose, and develop treatment plans within scope of practice
• Educate patients and families about health management and treatment options
• Support daily clinical operations efficiently and safely
• Contribute to a team-centered, patient-focused healthcare environment
• Engage in ongoing professional development and maintain licensure requirements

What You Need
Required:

• Vermont APRN license and certification
• Minimum 1 year of clinical APRN experience
• BLS certification
• Strong collaboration and communication skills

Preferred:

• Experience in primary care or outpatient settings
• Bilingual abilities are a plus
