As a Clinical Consultant Pharmacist at CareRx, you will maintain continuity of care by working closely with onsite physicians, nursing staff, and other healthcare providers. Your role involves executing clinical pharmacy activities following established standards and policies for long-term care and supportive living communities. Emphasizing collaboration, you will help enhance health outcomes and develop ongoing partnerships within the community. Key Responsibilities: • Act as a liaison between LTC homes and CareRx Pharmacy • Travel to LTC homes and retirement communities regularly • Conduct medication reviews and audits in serviced locations • Provide educational sessions on health topics • Maintain compliance with provincial regulations and standards Requirements: • Insured and licensed with the Ontario College of Pharmacists • Proven partnership skills in healthcare teams • Effective organization and time management • Familiarity with Kroll and Microsoft Office • Valid driver's license is required Make a real difference in the lives of seniors through expert pharmaceutical care and support within the GTA.
