Medicaid is required to be actively enrolled. Active State License

Reason for Coverage Needed:

In need of multiple providers due to a few doctors retiring and another provider going to a specialty program.

Start Date/End Date of Coverage required:

Immediate start date and no definitive end date as of now

Work Schedule (any flexibility?):

We are flexible but prefer full time but would be open to part time

Shifts Times:

Monday-Friday 8-5 at all locations.

Saturday 8-1 at our Monroe location is also available but not required.

Average Patients/shift?

Patient Ages/Percentage Breakdown:

I do not currently have all this for each location but can gather if necessary.
